Simplicity has one, number 5879, but I think it's pretty old. I did a quick google search and wasn't able to come up with anything for you. I know I could've used one, too, b/c my dd's gi was REALLY expensive!

A lady has it posted on iOffer (never heard of this site, but....). It says it's "expired" but maybe you can contact her and see if she still has it available?
I don't know if this store will have what you're looking for or not, but I've had it bookmarked forever because my brother wanted me to make him some Gi (sp?) pants.
